<DIV STYLE="text-align:Left;"><DIV><DIV><P><SPAN>Department of Health Primary Care Service Areas: Received from State Dept. of Health, December, 2019. Boundaries are based on census tracts. Primary care service areas were selected to describe the delivery of primary health services in the State of Hawai‘i through participation of multiple stakeholders in the early 1990’s. The rural health associations of the Hawai‘i, Maui and Kaua‘i Counties delineated rational service areas under their respective jurisdictions. The Needs Assessment Committee of the Primary Care Roundtable participated in the delineation of rational service areas....referred to as primary care service areas. Clustering of neighborhoods into these primary care service areas was intended to provide information below the county or island level with demarcation between adjacent neighborhoods. Census tracts were used in defining these areas due to their availability in census and vital statistic data. The size of the population in these areas, based on the 2010 U.S. Census data, varied from 170 individuals in Ni‘ihau and 2,291 in Hāna to 115,164 in Ko‘olaupoko. Thus direct comparisons in estimates between primary care service areas are limited.
